For your convenience, the following forms are available to be printed, completed and turned in to the Greenport Town Clerk's Office  for registration of children for the Greenport Playground Program which starts July 5, 2011.  A copy of current immunization record must be attached to the application, as mandated by NYS.  All children must have completed one year of kindergarten to be eligible.  Greenport residents are FREE,  non-residents will be assessed $100.00 per family.  Hours are 8:00 a.m. to 2:30 p.m. Monday through Friday.  Children must be picked up promptly at 2:30 p.m.  

Children must wear white playground t-shirts which may be purchased from the Greenport Town Clerk's office for $4.00 each.
